Cardiologist pay hits new high amid productivity decline

Cardiologist compensation reached an all-time high in 2024 despite falls in productivity, according to a new MedAxiom report.

For the 2025 “Cardiovascular Provider Compensation & Production Survey,”  the company asked 6,830 cardiology physicians and advanced practice providers across 232 programs about their compensation, productivity growth and case volume.

Here are eight things to know.

1. Full-time cardiologist compensation reached an all-time high in 2024, at a median of $694,954.

2. All subspecialties saw compensation gains despite a slight decline in median work relative value unit (wRVU) production.

3. Integrated cardiologists had a median compensation of $700,000 per full-time physician, while that of private practice cardiologists declined to $588,479 — the largest gap in over five years.

4. Cardiac surgeons had a modest compensation increase amid declining productivity, while vascular surgeons maintained near-peak compensation despite a steep drop in wRVUs.

5. APP-to-physician ratio increased to 0.75 in cardiology programs, while cardiac and vascular surgery programs had a decrease of APP support per surgeon.

6. Private practice APPs produced a median of 2,743 wRVUs, outperforming their integrated peers.

7. The largest patient panels were seen in 2024 at nearly 2,000 per full-time cardiologist. The percentage of new patient visits to total office visits fell to the lowest point in five years — 15.4%.

8. Cath lab volumes also declined for catheterizations and PCIs per 1,000 active cardiology patients.
