The at-home program provides cardiac rehabilitation exercises, nutrition counseling, support groups and social support services, according to an Oct. 18 news release from Tampa General. Patients can also connect with nurse navigators to answer questions and provide support through the care process.
The program is designed to go beyond the standard 12-week rehabilitation to provide ongoing, holistic support.
