Hospitals with highest, lowest return rates for heart failure patients

Bloomer, Wis.-based Mayo Clinic Health System—Chippewa Valley has the lowest number of hospital return days for heart failure patients in the nation, according to CMS data.

The “Unplanned Hospital Visit” database tracks provider data for hospital return days, unplanned readmission and measures of unplanned hospital visits after outpatient procedures. Hospital return days add up the days a patient was in an inpatient hospital unit, under observation or in an emergency department for unplanned care in the first 30 days after discharge. Hospitals with fewer than average days are likely arranging timely follow-up care, ensuring patients have proper medicines and releasing patients to proper post-acute care locations.

Becker’s analyzed the hospital return days for patients with heart attack for this listing. The current data was collected from July 2021 to June 2024, and updated May 13.
