Dover, Del.-based Bayhealth Heart & Vascular Institute has launched a congestive heart failure clinic to improve patient outcomes after discharge and reduce hospital readmissions.
The clinic will operate out of Bayhealth Cardiology facilities in Dover and Milford, Del., according to a Dec. 11 news release from the health system.
The clinic will schedule expedited appointments for patients diagnosed with heart failure to see cardiology specialists and pharmacists who will collaborate to help patients manage their condition. The clinic will also offer outpatient fluid reduction treatments, the release said.
